Before your appointment:
Close to the day of your booked appointment, you will receive a text message from our practice that contains a link to a https:// address, this is the most secure way to send us the requested information, required by us, in relation to current guidance and managing your care during COVID-19 pandemic. If for any reason you cannot access this link please click on BOTH the following links and complete BOTH forms, prior to your appointment.

At your appointment:
Attend for your appointment at the BACK of our practice. This is accessed from Riseley St. SK10 1BW.
Proceed on foot through the car park. (This is a staff car park only.)
Wait outside the back door in the marked yellow boxes under the shelter. The nurse will come to meet you when it is your appointment time.
Wear a face covering or mask.
Attend alone unless you are under 16 or need assistance or a carer.
